It is possible that you do not have the required codec installed. 

mp4 is a multimedia container format - you need to know what codec is used inside this file.  GSpot or MediaInfo can be used to analyse the video and tell you which codec is required.  GSpot will also tell you if the video file can be renderred using the buttons bottom left of the GSpot window.

Also do you have Quicktime installed or up to date? If not try installing the latest version and then importing the video into MEP.
