Simple Q : Is the "Request form" in the picture (top right) included in the "online content catalog" ?? It's possible I'm blind, but where do I find it.

What is the version number of Web Designer Pro X you are using?
What is the full name of the 'request form' widget as shown in the manual? A screenshot would help.
Did you try using the search in the Online Content Catalog for the form shown in the manual - you do need a valid Update service to access the Online Content Catalog.

From the image you posted that example form could be any one of the forms widgets listed in the Widgets guide information I linked to above.
Depending on what you want from the form, you may need to look at the various widgets to find the correct form for the purpose required.
If you have a Google/GMail account I would suggest you look at Google forms first.
If you are wanting the form for eCommerce then you need to be looking at the eCommerce section of the Widgets guide I linked to previously.
