What is this?  Nominet tag of the new provider - this one of the lines in the form. Do you know if Magix has a U.S. fax number?

This is the shortform name by which Nominet recognise a provider of .uk domains - you should get this from them or their help information on transferring domain name to them.

Hosting companies which host .uk domains are required to have a Nominet Provider tag.

Unless you are wanting a .uk domain name then, AFAICS, you do not need this information.

Cannot find FAX no for US offices.

I found some very helpful information after a long search on Magix.com

In order to transfer your domain, you will need to cancel it with us. This will then provide
you a transfer authorisation code that you can give to the new provider.

1. Download and print out the following cancellation form:
http://www.magixonline.com//uploads/MAGIX_ONLINE/us/dokumente/magix_domain_cancellation.pdf
(your mentioned document)

2. Make a digital or photograph of the form and create a ticket for MAGIX
Customer Services using the New request option above.

3. Use the option to
transfer/cancel the domain and attach the form to the ticket.

4. You should then
receive confirmation of the cancellation and you will receive your authorisation
code from our Domain Providers.

 5. Give this authorisation code to your new
provider and the transfer will be complete.
